I went into Leper's Smile with nothing to heal with except 1 scroll of "Cure Serious Wounds" [it was to test outcomes. It is NOT part of the solution] . This is 100% the issue. Wand of Zacharius is a magic wand in Pathfinder: Wrath of the Righteous. Found in Leper's Smile (Cave) at the Nasty Waifs gang hideout. Used to unlock the barrier to Zacharius's hideout in Lost Chapel. It's a literal sentient power if not quite alive. If I had to guess, Zacharius imprisoned it in the wand and used it as a "Drain Soul" wand which would explain all the souls inside it. You find out (not sure if this is Lich only or not) that this wand is Zacharias' phylactry. In other words: it contains his (very evil) soul.All right, I found the answer to my question. For anyone having the same issue: The handkerchief is being removed automatically after the boss fight in Leper's Smile. However, it is not needed. All you need is the perfume, for which you have to pass the perception check. Because at Paladin 5 you get a choice between Divine weapon and a horse. If you picked the weapon power, you'll need to respec. If some of her levels weren't in paladin, then she needs to do more paladin. You have to have an animal companion.
