You may not have noticed the quest marker when you completed the order hall campaign because the weapon altar is so far from the central order hall and no one goes to the altar there anymore now that artifact power is gone from the game. Make sure you have the achievement "A Glorious Campaign" for the specific character you are on. It is not account-wide. Also make sure you have the first two parts of the "Breaching the Tomb" achievement completed. Then, inside your class hall you'll have an npc that starts a quest chain that will result in you receiving the follower.

Champions of Legionfall is a step in the Broken Shore campaign. You have to clear the previous criteria of that campaign before Champions is offered.
